SWEEPSTAKES ENTRY PERIOD: NephCure’s Keys for a Cure Sweepstakes begins on 4/1/11 and closes on 12/8/11.
ELIGIBILITY: Open to legal residents of the 48 contiguous United States or the District of Columbia. Participants must be age 18 and older at time of entry. Employees of The NephCure Foundation (NephCure) are not eligible to participate. Immediate family members of paid NephCure staff and/or those living in the same household, whether related or not, are not eligible to participate. Sweepstakes is subject to all applicable federal, state, and local laws and regulations and is void where prohibited by law. Residents of Alaska and Hawaii not eligible for entry.
HOW TO ENTER: Tickets for Sweepstakes can be obtained at NephCure Foundation events online at www.nephcure.org/KeysforaCure, or by calling 1-866 NephCure, or by mailing in a request to The NephCure Foundation at: The NephCure Foundation, 15 Waterloo Ave. Berwyn, PA 19312, Attn: Sweepstakes. Request must include a return envelope with paid postage.Tickets will not be deemed valid if all fields are not completed. Completed tickets can be returned in person or by mail to: The NephCure Foundation, Attn: Sweepstakes, 15 Waterloo Ave. Berwyn, PA 19312. Reply must be postmarked by 12/5/11 and received by 12/7/11 NephCure is not responsible for late, lost, incomplete, damaged, illegible or postage due entries. All submissions become the property of NephCure and will not be returned or acknowledged. Odds of winning depend on the number of eligible entries received.
No portion of sweepstakes donation is tax-deductible.
PRIZE VALUE: Three prizes will be awarded to winners chosen at random by a third party selection committee.
First Prize: One First place (grand) prize will be awarded. First place is redeemable for 2011 Ford Fusion I4 S, Ford Escape XLS, or Ford Mustang V6 Coupe. Winner may also choose to accept $15,000 cash paid in one lump sum. Prize is non-transferable except to a surviving spouse. Winner is responsible for all federal, state, local, sales and income taxes associated with winning prize. No test drive required to win.
Second Prize: One Second place (2nd) prize will be awarded. Second place entitles the winner to a vacation gift certificate in the amount of $2,500.00 from travel company determined at later date. Gift certificate subject to rules, regulations, space and availability imposed by travel company, associated industries such as hotel, airlines, cruise companies, or any other travel company associated with vacation chosen by winner. NephCure or any involved companies are not responsible for costs incurred by winner should trip be cancelled, rescheduled, or delayed. Should winner have to cancel trip after arrangements are made, NephCure has no further obligation to winner. Should weather affect ability of winner to travel at scheduled date, NephCure has no further obligation to winner. Travel subject to all governmental regulations. Winner and guest are responsible for obtaining any necessary travel documentation such as photo IDs, passports, or visas prior to travel. NephCure and any involved parties not liable for any inquiry that should occur on trip. All prizes non-transferable except to surviving spouse. Winner responsible for all federal, state, local, sales, and income taxes associated with wining prize.
Third Prize: One Third place (3rd) prize will be awarded. 3rd place entitles winner to 47” LCD Television valued at $1,100. Winner is responsible for all federal, state, local, sales and income taxes associated with winning prize. All prizes non-transferable except to surviving spouse. Additionally, NephCure will pay $100 towards shipping of television. Any shipping cost over $100 is responsibility of winner. NephCure is not responsible for any damage to television after shipment.
RANDOM DRAWING: Potential Winner (“winner”) will be selected in a random drawing from all eligible entries received during the Sweepstakes entry period of April 1, 2011- December 8, 2011. Random drawing will be conducted in a live drawing on 12/8/2011 by an independent judging organization (“judges”) whose decisions are final. Winner does not have to be present at live drawing to win. Winners are eligible to be chosen multiple times, as drawing is random. Chosen winners entries will be removed from next round of drawing. Winner will be notified by mail and will be required to complete and return an Affidavit of Eligibility and Liability and a Publicity Release within 10 days of date specified on notification, or an alternate winner will be randomly selected. If winner cannot be contacted, or if Affidavit/Release is returned as undeliverable, or if the selected winner cannot accept or receive the prize for any reason, or he/she is not in compliance with these Official Rules, the prize will be forfeited and it will be awarded to an alternate winner in a separate random drawing.
USE OF LIKENESS: Entry and acceptance of prize offered give NephCure and affiliated partners permission to use winner's name, photograph or other likeness for the purpose of promotion on behalf of The NephCure Foundation, unless prohibited by law.
CONDITIONS AND LIMITATIONS OF LIABILITY: By participating, entrants agree to be bound by Official Rules of Keys for a Cure Sweepstakes and the decisions of NephCure and independent judges. All decisions of judges will be final in all respects. Failure to comply with these Official Rules may result in disqualification from Keys for a Cure Sweepstakes. NephCure reserves the right to permanently disqualify any person it believes has intentionally violated these Official Rules. Participants release NephCure, David Ragan Ford, and any involved companies and individuals, including employees, officers, directors and agents, from all liability for loss, harm, damage, injury, cost or expense whatsoever including without limitation, property damage, personal injury and/or death which may occur in connection with, preparation for, travel to, or participation in Sweepstakes, or possession, acceptance and/or use or misuse of prize or participation in any Sweepstakes-related activity. Winner assumes all liability for any injury, damage, or death directly or indirectly caused by use of prizes received in Keys for a Cure Sweepstakes. NephCure is not responsible for any error in reprinting of promotional items not produced directly by The NephCure Foundation headquarters at 15 Waterloo Ave. Berwyn, PA 19312. NephCure is not responsible for any error in the announcement of the prizes. NephCure is not responsible for error, omission, interruption, deletion, defect, delay in operations, transmission of information online, through telephone equipment, computers, or hardware or software of any kind. NephCure not responsible for any theft, destruction or unauthorized access to or alterations of entry materials.
